ALBANY – The Salvation Army Care Management, which serves children in 48 New York counties, is now sharing patient information through Hixny. The care management agency was created in 2016 to serve Medicaid-eligible patients under age 21 who have two or more chronic health conditions, serious emotional disturbance or mental illness, complex trauma or HIV/AIDs. It coordinates medical, mental health and social support services for these individuals while working to address family needs such as employment and housing.

Salvation Army Care Management will be using Hixny to safely and securely share information about its patients. It will also help parents learn to navigate patient portals, like Hixny For You.

This is only possible because New York changed its rules governing consent for the sharing of minor health records. Until recently, parents could not consent to share health data about children ages 10 through 17. Now, parents may sign consent forms for all children under 18. Over the past few months, pediatric practices throughout the region have connected to Hixny and begun to share their patients’ information.

The agency can use Hixny to share information across its 48-county footprint through Hixny’s connection to the Statewide Health Information Network for New York (SHIN-NY).

The Salvation Army Care Management connection adds to Hixny’s continuous advancement of meaningful exchanges of health information in the region. Through an increasing number of connections, and with growing patient consent, the region’s healthcare community and counterparts across the state can share the records that support better coordination of care for all New Yorkers.

Hixny, a not-for-profit healthcare collaborative, is a national leader in population health management support. Hixny utilizes secure technology to provide individuals, healthcare providers, payers and public health entities with the ability to share and use real-time electronic health records. This coordination of data enhances care for more than 1.7 million people in New York’s Capital District, Northern New York and the Mohawk Valley.

Hixny will be performing extended maintenance beginning Thursday, December 31 through Friday, January 1. During this time, all services
will be impacted and unavailable—including access to the provider portal.
