Case managers at Bassett Healthcare Network’s primary care sites are now receiving logic driven alerts from Hixny.

The six case managers, who support a total of 75 providers in their care delivery, began receiving the notifications in late 2016. What makes there alerts more effective is that there is logic in place to deliver each case manager alerts for only those primary care providers they assist.

In November, Hixny delivered a total of 909 alerts to the Bassett case managers. This number jumped to 1,783 alerts in December. These include notifications of emergency department admissions, ED discharges, ED summary reports, inpatient admissions, inpatient discharges and inpatient summary reports. The notifications are delivered to the case manager’s email in the Hixny360 provider portal.

The alerts enable the Bassett primary care case managers to improve care coordination, by being proactive and reaching out to the patients for follow-up. This allows the 75 providers to focus more effectively their energy on patient care.

Hixny will be performing extended maintenance beginning Thursday, December 31 through Friday, January 1. During this time, all services
will be impacted and unavailable—including access to the provider portal.
