ALBANY, N.Y. – Hixny connected two organizations to the community-wide health information exchange (HIE) in August, further expanding the availability of patient records to the statewide healthcare community and to individual healthcare consumers.

The new connections are:

  • The Teresian House Nursing Home,a nonprofit rehabilitation and assisted living facility in Albany, N.Y.
  • Price Chopper Pharmacies, a supermarket network that also includes:
    • Market 32 pharmacies
    • House Call pharmacies

The Teresian House Nursing Home is contributing continuity of care documents. Price Chopper Pharmacies is contributing prescription status information from their billing system, indicating when a prescription is filled, as well as when a patient picks up that prescription or when it is returned to the shelf.
