As you may have heard, earlier this week Governor Hochul announced the much-anticipated approval of New York’s Medicaid 1115 Waiver Amendment.
Whether or not you will realize any impact from the waiver, you are in a position to help advance health equity, reduce health disparity and address substance use through Hixny. Here’s how:
- Complete social needs assessments using the most widely-accepted screening tool available. Log into Hixny to complete in-person with patients or send it to them by email or text to complete on their own.
- Review your patients’ social needs history, keeping you connected to their complete care. Responses to assessments completed through Hixny are recorded and made available through Hixny to you and all members of the patients’ care team.
- Make referrals for help addressing identified social needs. If a need is identified, refer your patient for help then and there.
- Search the state’s Prescription Monitoring Program (PMP) tool. To help prevent abuse of prescription medications, search PMP from directly within a patient’s Hixny record.
