A Smarter Snapshot in the EHR

Using SMART on FHIR app technology, Hixny makes patient records available to providers within several leading EHRs. But we go farther than others in the field.

By applying advanced analytics to patient data, we present providers with the next best steps in care to improve outcomes. Alongside the standard information of importance in a patient encounter, Hixny users see gaps in care—such as necessary preventive screenings—and areas of risk to address during the visit.

Improving outcomes requires attention to more than clinical conditions. That’s why Hixny is focused on making strides toward heath equity by giving providers the tools to identify and address health-related
social needs (HRSN) within their current workflow.

Within the Hixny app, providers can view social needs history, complete a screening and—when necessary—make an immediate referral for social services through our partnership with Healthy Alliance.
