Hixny Makes Social Screenings Easy

Beginning in January 2025, the New York State Department of Health calls for all Medicaid patients to be screened for health-related social needs (HRSNs). Under the state’s CMS 1115 waiver amendment, this requirement is intended to help increase health equity by connecting patients to community-based organizations that will help patients find relief from these needs—so they can focus on improving their health.

This includes a modified version of the CMS Accountable Health Communities (AHC) screening tool that gains patient consent to screen and share results with organizations that can help them if needs are identified (“Question Zero”).

Simple for Providers

Screening is available alongside the patient’s clinical data in their Hixny record snapshot, in the app as well as the provider portal. This means you can conduct a screening without leaving your EHR workflow.

Questions and answers (data) are available as required—no extra integration work needed with Hixny or the social care network (SCN).

Responses to the screening are automatically and immediately shared with the SHIN-NY, SCN, insurers, and other mandated organizations.
