Hixny For You is a patient portal that allows you to view your community health record, securely and online. You no longer need to provide your medical history at every new doctor’s visit – simply transmit your record to your providers through the portal or download and take with you to your next appointment.

Why wouldn’t you want your doctor to have your comprehensive medical history, rather than just what they have on file? This enables them to provide you with higher quality, more efficient and more convenient healthcare.

A community health record is comprised of your medical history with information aggregated from connected organizations such as hospitals, physicians and specialists, from across New York State. This information may include test results, medication history, immunizations, allergies, and much more.

What should you do if you feel there are inaccuracies in your electronic medical records?

If you believe that any of the information within your secure electronic medical record is incorrect, please refer to our participant list and contact your care provider to review your records. Hixny does not modify or update patient records, this must be done by your provider.
